興大獸醫教學醫院 首創自體前十字韌帶置換手術
前十字韌帶斷裂是小動物常見的外科病例，中興大學獸醫教學醫院近期採用創新的自體移植韌帶方式，成功完成8件犬隻手術，以動物體自身的韌帶來取代斷裂的韌帶，適應性好且復原期縮短。

有一隻8歲的奶油法鬥，11.5公斤，左後肢不負重，診斷為前十字韌帶斷裂。經興大獸醫教學醫院手術治療後，兩個星期後即能負重，正常走路。
興大獸醫教學醫院教授李衛民表示，前十字韌帶斷裂臨床症狀從肢體跛行到不負重都有可能發生，最常見的症狀是整隻腳懸空，不敢著地，但有時候症狀輕微，所以主人往往會忽略。由於膝關節受力不平衡，最終導致膝關節的半月板軟骨受損甚而破裂。
依據體重的考量，前十字韌帶斷裂的修復方法有很多種，包括脛骨平台位移(TPLO)、脛骨粗隆前移術(TTA)、改良式的TTA 進階手術(MMP)、膝關節囊外固定、膝關節囊內固定等。在這些手術中，只有膝關節囊內固定，傾向於模擬膝關節內前十字韌帶的走向，但仍是以外來物取代原來的韌帶，而TPLO、TTA與MMP皆是先對脛骨近端或是脛骨粗隆進行切割，以利調整膝關節股骨與脛骨關節面對合的位置，減少對膝關節軟骨的衝擊，再以骨板固定。
前十字韌帶的檢查，主要是靠抽屜實驗與脛骨壓迫實驗，於抽屜試驗檢查時，若發現股骨與脛骨有錯動，就可以判定是前十字韌帶斷裂(幅度大)或是鬆弛(幅度小)；於脛骨壓迫實驗檢查時，當脛骨做出往上壓動作時，發現脛骨會往前滑動，就可以判定是前十字韌帶斷裂。前十字韌帶斷裂，藥物的治療只能緩解疼痛，無法根本解決此疾病，因此最好的方式還是進行手術。經過近年來的觀察與臨床經驗，目前認為韌帶置換手術，對10公斤左右或以下犬隻是最好的選擇。
由李衛民所率領的外科團隊，應用改良式的膝關節囊內固定，以動物體自身的韌帶來取代斷裂的韌帶，好處是自身的韌帶，不會產生排斥，同時韌帶伴有血管，不會變成死物。目前已完成的病例體重大都在10 公斤左右，最重到達16公斤，而且動物恢復的狀況良好，另觀察到體重越重，限制活動與恢復的時間會有所延長。若主人對家中的毛小孩，是否有前十字斷裂的疑慮，也歡迎預約進行門診諮詢。
